How to begin Physical Therapy at Pilates People

If you plan to privately pay for Physical Therapy, please call our office for rates and to make an appointment. Please Download and complete the New Patient Paperwork to bring to your first PT appointment. DOWNLOAD HERE
If you would like Physical Therapy at Pilates People to bill your medical insurance company for your treatment we will need a written prescription for physical therapy from your medical doctor. We recommend that you contact your medical provider and ask for a written prescription, or visit your physician and be evaluated prior to you first physical therapy visit.

Our office staff will gladly contact your insurance company and let you know what your coverage is for Physical Therapy. When you call us, please have your insurance card handy. Our front office staff will ask some important questions regarding insurance verification, coverage, and personal information in order to verify your benefits.

Your first visit:
Once you have scheduled your first appointment here are a few items of information that will help you. Physical Therapy at Pilates People is a full service clinic and we will do our best to work with your schedule and accommodate your needs. Our spa style clinic will help to make your therapy a comfortable, stress-free and pleasant experience.
- Your evaluation will take approximately 1 Hour. Please arrive 15 minutes prior to your first scheduled appointment to ensure that all your paperwork is completed and correct.
- Please dress in a way that will allow your therapist the best opportunity to evaluate you effectively. We recommend that your wear comfortable clothes and bring the shoes that you wear most often.
- If you have any relevant paper work i.e. MRI reports, x-rays, surgical reports etc., please bring it to your first visit.
Please bring your calendar so that you can schedule follow up appointments.
